To aid in the data submission and review process, IPRO suggests the naming convention below. The Clinical Data Portal will accept any valid file, however, the use of naming conventions will aid in correcting errors or in the resubmission of valid files. 

For single facility submissions

sdc_id_yyyy-mm-dd_version.csv or sdc_id_yyyy-mm-dd_version.txt 

WHERE

sdc = project identifier (do not change),
id = four digit permanent facility identifier (PFI), or six digit Pellucid ID,
yyyy-mm-dd = date of file creation (not date of file submission to Clinical Data Portal), and
version = single digit 0-9 version of the file (default to "0", increment if versioned on the same day).

For multiple/group facility submissions

sdc_groupid_yyyy-mm-dd_version.csv or sdc_groupid_yyyy-mm-dd_version.txt

groupid = Clinical Data Portal group identifier (available from within the portal),
yyyy-mm-dd = date of file creation (not date of file submission to Clinical Data Portal), and
version = single digit 0-9 version of the file (default to "0", increment if versioned on the same day).
